Before starting
Prepare your Social Security number, birth details, direct-deposit information, names and dates for marriages, information about children when relevant, and recent work details. Medical information should include providers, facilities, dates, medicines, tests, and conditions.
Steps
- Open Social Security’s official disability application page.
- Check whether you are applying for SSDI, SSI, or both.
- Complete the benefit application and the Adult Disability Report when required.
- Provide authorization for SSA to request medical records.
- Save your confirmation and respond to requests from SSA or the state Disability Determination Services office.
