Report changes to Medicaid or CHIP

Use the reporting method approved by the state program, give the effective date, submit requested proof, and keep the confirmation.

Report the change to the program that manages the case

Changing an address with USPS, an employer, or another benefit program does not automatically update a Medicaid or CHIP eligibility record.

1. Identify the responsible agency

Use the state or territory where the person is enrolled. Contact the Medicaid or CHIP agency shown on the eligibility notice or member materials. If the application is maintained through HealthCare.gov, follow the instructions in the Marketplace account and the state notice.

2. Check which changes must be reported

State rules and reporting periods differ. Common examples include an address or contact change, a move to another state, household membership, income or employment, pregnancy, other health coverage, immigration information, or a change affecting disability-based eligibility.

3. Use an approved reporting channel

Report through the state portal, phone line, mail, local office, or another method the agency authorizes. Do not send sensitive information through an unofficial email address or a third-party website.

4. Provide the effective date and proof requested

State when the change happened or will happen. Upload or send only the evidence requested, such as a new address, pay information, loss of other coverage, or a household record.

5. Save confirmation and read the new notice

Keep the submission receipt, reference number, document list, and copies. A reported change can affect only some household members or can transfer information to another coverage program.

Moving to another state

Medicaid does not generally transfer as a single national policy. Contact the current program about the move and apply in the new state when instructed. Avoid assuming that coverage, providers, or managed-care enrollment will continue unchanged.
