Start with the notice
The cessation notice identifies the affected program, effective date, appeal route, office, and deadlines. Do not use an ordinary initial-denial checklist without confirming that it covers a medical cessation.
Notice and deadline file
- Complete cessation notice and every enclosure.
- Envelope or electronic delivery record.
- Notice date and actual receipt date.
- Calendar entry for the general 60-day appeal deadline.
- Separate calendar entry for the SSA-792 continuation deadline—currently 15 calendar days from the notice date on the form.
- Written explanation and proof if either filing will be late.
Appeal forms
- SSA-789 (04-2026): request reconsideration of a medical disability cessation and choose whether to appear at the disability hearing.
- SSA-792 (07-2026): elect or decline statutory benefit continuation.
- Any current Disability Report—Appeal or medical-release form requested by Social Security.
- Interpreter language and accommodation needs.
- Representative appointment information, if applicable.
Medical and functional updates
- Current diagnoses and symptoms.
- Names and contact information for treating professionals and facilities.
- Recent visits, hospitalizations, tests, therapy, and medication changes.
- Side effects and assistive devices.
- Limits on standing, walking, sitting, lifting, concentrating, interacting, self-care, and daily activities.
- Work activity, unsuccessful work attempts, accommodations, and reasons work ended or changed.
- Evidence that directly addresses the reason stated in the cessation notice.
Benefit-continuation decision
- Confirm which cash benefits and Medicare choices appear on SSA-792.
- Understand that continued cash payments can become an overpayment if the appeal is unsuccessful.
- Keep reporting work, income, living arrangement, and dependent changes.
- Complete the good-cause section if the election is late.
- Plan to make a new election if an unfavorable reconsideration is appealed to an administrative law judge.
Hearing preparation
- Choose whether you and/or a representative will appear.
- Prepare a short timeline of the disability, treatment, and changes since the last favorable decision.
- List the specific findings in the cessation notice that you dispute.
- Submit evidence as early as possible and keep a copy.
- Notify the hearing unit of address changes, hospitalization, interpreter needs, or scheduling conflicts.
- Attend any consultative examination Social Security schedules.
Filing proof
- Copy of each signed form and attachment.
- Online confirmation or upload receipt.
- Fax transmission report.
- Tracked mailing receipt.
- Office-stamped copy for an in-person delivery.
- Name or identification number of an employee who confirms receipt.
After filing
- Confirm that both the appeal and continuation election were received.
- Watch for requests for records, examinations, hearing notices, and deadlines.
- Check that continued payments match the election and report any problem promptly.
- Read the reconsideration decision immediately; a hearing request and renewed continuation election have separate deadlines.
- Request qualified legal or representative help when the record, overpayment risk, or hearing issues are complex.
