Social Security disability hearing checklist

Organize the hearing request and the later preparation steps without entering personal or medical information on WhatDoIFile.

Before filing

  • Read the complete reconsideration notice.
  • Confirm that a hearing before an administrative law judge is the stated next appeal level.
  • Record the notice date, actual receipt date, and filing deadline.
  • Choose the official online, upload, mail, or local-office channel.

Request materials

  • HA-501, Request for Hearing by Administrative Law Judge.
  • SSA-3441, Disability Report—Appeal.
  • SSA-827 authorization, following SSA's signature instructions.
  • SSA-1696 only when appointing a representative.
  • A written late-filing explanation and supporting proof if the deadline may have passed.

Medical and vocational updates

  • New or changed diagnoses, symptoms, and limitations.
  • Current providers, facilities, treatment dates, tests, and medicines.
  • Emergency visits, hospital stays, therapy, and assistive devices.
  • Changes in work activity, job duties, attempts to work, education, or training.
  • People or organizations holding relevant records that are not yet in the file.

Filing proof

  • Complete copy of the request and attachments.
  • Online confirmation, upload receipt, fax report, or mailing proof.
  • Date submitted and office or portal used.
  • Any reentry number or reference number stored securely.

After the request reaches the hearing office

  • Save the hearing office contact information from each notice.
  • Update mailing address, phone, and email when they change.
  • Read the Notice of Ways to Attend a Hearing.
  • Return agreement or objection forms by the stated date.
  • Check appeal status through the official SSA service.

Evidence and case-file review

  • Review the evidence already in the claim file.
  • Submit or identify written evidence as early as possible.
  • For a disability hearing, track the general 5-business-day evidence rule.
  • Keep proof of every evidence submission and avoid unnecessary duplicates.

Hearing arrangements

  • Date, time, time zone, and method of appearance.
  • Interpreter language, including American Sign Language, when needed.
  • Accessibility accommodation requested from the hearing office.
  • Representative and witness contact information.
  • Immediate contact with the hearing office if attendance becomes impossible.

After the hearing

  • Keep the written decision and envelope or delivery record.
  • Compare the decision with the issues and evidence considered.
  • Read Appeals Council instructions immediately if you disagree.
  • Do not assume a new application preserves an appeal deadline.
