- Expected annual household income when the application uses Marketplace or MAGI information.
- Tax-filing relationships when requested.
Identity and status information
- Social Security numbers for applicants when required and available.
- Citizenship or eligible immigration-status information for people requesting coverage.
- Identity documents if the state asks for them.
Other coverage and special circumstances
- Current health-insurance information.
- Pregnancy, disability, age, Medicare, long-term-care, or former foster-care information when relevant.
- Resources or assets when applying through a non-MAGI eligibility pathway that considers them.
After filing
- Application copy and confirmation number.
- Every verification request and your response.
- Eligibility notice, effective date, plan-selection information, and appeal instructions.
