Medicaid renewal and coverage transition checklist

Track the notice, deadline, requested information, submission proof, final decision, appeal rights, and any transition to other health coverage.

Use this checklist for a renewal, a request for information, or a coverage-ending notice

The state notice controls the required action. This list helps organize the case but does not replace the jurisdiction's form, deadline, or appeal instructions.

Before the renewal arrives

  • Confirm the Medicaid or CHIP agency has the current address, phone, email, and preferred language.
  • Keep the member ID, eligibility notices, portal login, and authorized-representative information secure.
  • Organize recent household, income, other-coverage, pregnancy, disability, and immigration records that may be relevant.

When the notice arrives

  • Record the notice date, response deadline, effective date, and each household member named.
  • Identify whether the state renewed automatically, needs information, denied an application, or plans to end coverage.
  • Use only the portal, phone, mail, office, or upload channel listed by the state.
  • Return the signed form and requested evidence without adding unnecessary sensitive records.
  • Save screenshots, tracking, confirmation numbers, and a complete copy.

After submitting

  • Check the portal and mail for another request.
  • Verify that uploaded or mailed documents were received.
  • Read the final notice for each person and confirm the coverage date and plan instructions.
  • Report later changes through the state-approved process.

If coverage is denied or ends

  • Compare the reason with the information submitted.
  • Correct errors and ask whether reconsideration is available.
  • Follow the notice to request a fair hearing before the applicable deadline.
  • Ask whether benefits can continue during the dispute and what action is required.
  • Explore Marketplace, employer, Medicare, CHIP, or other coverage promptly.
  • Keep medical appointments, prescriptions, and provider contact information organized during any transition.
