Do not delay filing solely because every document is not ready. Submit through the state process and respond to the specific verification notice.
Household and identity
- Legal names, dates of birth, and relationships of household members.
- Current home and mailing address and reliable contact information.
- Social Security numbers or other information requested for people applying.
- Identity documents or account-verification information requested by the state.
Income and resources
- Recent pay statements and employer information.
- Unemployment, Social Security, pension, child support, self-employment, or other income records that apply.
- Bank or resource information when the state application requests it.
- Information about changes or income that has ended.
Expenses that may matter
- Rent or mortgage and utility costs.
- Dependent-care expenses needed for work, training, or education.
- Legally owed child support paid outside the household.
- Allowable medical expenses for an elderly or disabled household member.
Immigration and authorized representatives
Provide only the information requested for people applying for benefits. If another person will apply or communicate for the household, prepare the state’s authorized-representative information.
After filing
- Confirmation number or stamped application copy.
- Interview date and contact instructions.
- Verification deadline and proof of each submission.
- Copies of approval, denial, EBT, reporting, and recertification notices.
