Do not postpone contact because you lack an item on this general list. Ask the WIC agency what it accepts and whether it can obtain or verify information another way.
1. Find the correct agency
- Your current state or U.S. territory.
- A tribal WIC agency, when applicable.
- The local clinic or office assigned by that agency.
- The agency’s application channel and appointment instructions.
2. Applicant information
- Names and dates of birth.
- Current address and contact information.
- Identity records accepted by the agency.
- Pregnancy, postpartum, breastfeeding, infant, or child information relevant to the application.
3. Income or program participation
- Recent income information requested by the agency.
- Household details used under local instructions.
- Proof of Medicaid, SNAP, or TANF participation when it may support income eligibility.
- Any explanation needed for irregular, seasonal, or recently changed income.
4. Certification and nutrition assessment
- Appointment date, format, and people who must attend.
- Health, growth, pregnancy, feeding, or nutrition information requested.
- Records or measurements the clinic asks you to bring.
- Interpreter, disability accommodation, transportation, or scheduling needs.
5. After the decision
- Eligibility notice and appeal information.
- WIC card or benefit access instructions.
- Approved-food information and benefit period.
- Nutrition appointments and recertification date.
- How to report address, household, income, or pregnancy changes.
