Supplemental Security Income (SSI) application checklist

Organize the records SSA may request, confirm the application route, protect sensitive information, and keep proof of filing.

Organize the application, but do not postpone it just to build a perfect file.

SSA says the list is not all-inclusive and that one document may sometimes substitute for another. The agency can tell you what is required and may help obtain difficult records.

Application route

  • Official SSI application page opened.
  • Online eligibility conditions checked.
  • Telephone or local-office appointment arranged when needed.
  • Appointment confirmation and possible filing date saved.

Identity and status

  • Social Security number or information needed to request one.
  • Proof of age when required.
  • Citizenship or qualifying noncitizen documents.
  • Current mailing address, residence address, and reliable phone number.

Income and resources

  • Recent pay stubs or self-employment records.
  • Award letters, bank statements, court orders, or receipts showing other income.
  • Statements for checking, savings, certificates of deposit, stocks, mutual funds, or bonds.
  • Information about property, vehicles, insurance policies, burial arrangements, and other items SSA asks about.

Living arrangements and household

  • Lease, rent receipt, deed, or property-tax record.
  • Household members and dates of birth.
  • Rent, mortgage, utilities, and household contributions.
  • Marriage and separation information when applicable.

Medical information for disability or blindness

  • Doctors, clinics, hospitals, therapists, and approximate treatment dates.
  • Prescription and nonprescription medicines.
  • Medical reports already available, without delaying the application to obtain a complete record.
  • Conditions and how they affect daily activities and work.

Work and education

  • Employers, job titles, dates, hours, pay, and duties for relevant work.
  • Education, training, and school attendance when relevant.
  • Information about work changes while the application is pending.

Submission and follow-up

  • Current Form SSA-8000-BK edition verified when the paper form is used: 09-2024 as reviewed.
  • Receipt or online confirmation saved.
  • Copies of documents kept.
  • Requests for interviews, examinations, or additional records answered.
  • Changes in income, resources, address, household, or living arrangements reported.

Privacy check

Do not type Social Security numbers, bank details, health information, immigration records, or a full address into WhatDoIFile. Provide them only through the official SSA channel being used for the application.
