Unemployment claim document and information checklist

Gather the information most state applications request, then check the exact instructions for the state where you worked.

Requirements vary by state

This is a preparation list, not a substitute for the state application. Use the official state portal for the final requirements.

Identity and contact information

  • Legal name, mailing address, telephone number, and email.
  • Social Security number or another identifier accepted by the state.
  • Identity-verification documents or account credentials requested by the state.
  • Alien registration or work-authorization details when applicable.

Employment and wage history

  • Names, addresses, and telephone numbers for recent employers.
  • First and last dates worked for each employer.
  • Work location, job title, hours, and earnings information.
  • Federal employment, military service, union, or out-of-state work records when applicable.

Reason the work ended

  • Layoff, discharge, resignation, reduction in hours, strike, leave, or another reason.
  • The last day worked and any severance, vacation, pension, or other pay.
  • Documents or messages that accurately explain the separation if the state asks.

Payment and follow-up

  • Bank routing and account numbers if choosing direct deposit.
  • A secure place to store the confirmation number and correspondence.
  • A calendar for weekly or biweekly certifications and deadlines.
  • Work-search records when required by the state.
