The non-applying parent or guardian signs Form DS-3053 in front of an authorized notary and provides a photocopy of the identification shown to that notary.
When this form fits
Use Form DS-3053 when the child is under 16, two-parent or two-guardian consent applies, and one parent or guardian cannot attend. It is not a substitute for proof that the adult at the appointment is the child's parent or guardian. Different evidence may apply when one person has sole legal authority or the other parent's consent cannot be obtained.
Prepare the current form
- Download Form DS-3053 directly from the Department of State.
- Enter the child's name and date of birth exactly as they appear on the passport application and citizenship evidence.
- Identify the passport product or products being authorized.
- Do not sign until the authorized notary instructs you to sign.
- Show the notary a valid government-issued photo ID and complete the notarization.
- Make a legible photocopy of the front and back of the same ID presented to the notary.
Timing and delivery
The Department of State requires Form DS-3053 and other notarized consent statements to be submitted within 90 days after signing or notarization. Send the completed original consent and ID copy to the applying parent or guardian so they can submit them with the child's DS-11 application. A scan used for planning does not replace the documents required at the appointment.
If the signer is outside the United States
Check the country-specific instruction before notarizing. The Department of State says a parent or guardian abroad may need to use a U.S. embassy or consulate, and its current form identifies countries where local notarization is not accepted. Do not assume a remote or foreign notarization will qualify.
What happens at the appointment
The child and the applying parent or guardian appear in person with Form DS-11, citizenship and parental-relationship evidence, identification and copies, the photo, fees, and the consent package. A passport authorizing officer may request more evidence. Form DS-3053 authorizes passport issuance; it is not a general travel-consent letter or a custody order.
If consent cannot be obtained
Review the official child-passport page for evidence of sole authority or Form DS-5525 for special or exigent family circumstances. The Department of State evaluates that route case by case, and submitting it does not guarantee issuance. Consider qualified legal help when custody rights are disputed.
