Direct answer
Your reservation and travel documents should use the same name. If you will travel under your new legal name, request a replacement passport using DS-5504, DS-82, or DS-11 as your circumstances require.
Which form applies?
- Use DS-5504 by mail when both the legal name change and passport issuance were less than one year ago and you can submit the required name-change evidence.
- Use DS-82 by mail after that period if the passport meets all renewal conditions and you can document the name change.
- Use DS-11 in person if you do not qualify for either mail route.
Do not rely on the old and new names being accepted together
Airlines, border authorities, and destination countries control their own document checks. Before booking, confirm the name required by the carrier and destination. A marriage certificate or court order is not a guaranteed substitute for a passport that matches the reservation.
If your address changed but your name did not
You do not need to replace an already-issued passport only because your home address changed. If a passport application is still being processed and the mailing address changes, follow Passport Services' current contact instructions.
