Start with the SSA questionnaire
SSA now lets people begin many original and replacement requests online, then complete document review through the route shown by the questionnaire.
Choose the correct request
- Original number and card: for a person who has never been assigned an SSN.
- Replacement card: when the card is lost, stolen, or damaged and no record change is needed.
- Corrected card: when your legal name or other information on the SSA record must change.
Documents depend on the request
SSA generally requires original documents or copies certified by the issuing agency. Identity evidence is required; an original SSN request can also require evidence of age and U.S. citizenship or lawful immigration status.
Cost and delivery
SSA does not charge for a Social Security card. After completing a replacement request, SSA states that the card is generally mailed within 5 to 10 business days.
