LIHEAP application checklist

Use this as a preparation list, then replace it with the exact instructions from the LIHEAP office serving your state, territory, tribal area, or locality.

Local instructions control

Do not delay an urgent crisis application while trying to collect every item below. Contact the office, report the emergency, and ask which missing records can be supplied later.

Program and deadline

  • Correct state, territory, tribal, or local LIHEAP office identified.
  • Service area confirmed for the home address.
  • Regular or crisis application period checked.
  • Authorized filing channel, appointment, and deadline recorded.
  • Language access, disability accommodation, or representative procedure requested if needed.

Applicant and household

  • Applicant’s legal name, date of birth, contact details, and identification.
  • Names and requested identifiers for everyone living in the household.
  • Relationship and household-composition information.
  • Citizenship or eligible-status records only when the local program requests them under its rules.
  • Proof of participation in another benefit when it may simplify income eligibility.

Residence and housing arrangement

  • Current lease, mortgage statement, property-tax record, shelter statement, or other accepted proof of residence.
  • Landlord or property-manager contact information.
  • Explanation of utilities included in rent, shared meters, submetering, or a utility account held by another person.
  • Housing-subsidy and utility-allowance notices when applicable.

Income and financial information

  • Pay statements and employer information for the period required locally.
  • Benefit, pension, unemployment, child-support, self-employment, or other income records requested by the program.
  • Zero-income statement or support explanation if the office uses one.
  • Household-size and deduction information required by the local plan.

Energy account and crisis proof

  • Recent electric, gas, fuel-oil, propane, wood, or other home-energy bill.
  • Account number, vendor name, service address, and account-holder name.
  • Disconnect notice, restoration amount, past-due notice, empty-tank statement, or crisis documentation.
  • Landlord notice or rent statement when energy costs are indirect.
  • Permission or account-access form if the bill is not in the applicant’s name.

Submission and follow-up

  • Application reviewed for signatures, dates, and complete answers.
  • Copies or scans saved before submission.
  • Confirmation number, receipt, appointment, or mailing proof retained.
  • Verification deadline and contact method recorded.
  • Written decision, benefit amount, vendor payment, and appeal instructions reviewed.
